Baptist Health PACE provides medical programs of all inclusive care for elder adults, ages 55 and older. The goal of PACE is to help seniors with serious medical issues remain living independently at home as long as possible, instead of going into a nursing home. Services provided include: primary, acute and preventative medical care, rehabilitative therapies, in-home care services, transportation to medical appointments, an adult day center and much more. If enrolled in PACE, you receive all your health care services exclusively from a dedicated Care Team that includes a primary care physician, nurses, therapists, social workers, personal care aides, and other professionals that oversee all aspects of your care. The Care Team works with you and your caregiver to determine what services will best meet your goals of care. Each person's plan of care is individualized. Baptist Health PACE has two locations, one in North Little Rock that serves Pulaski, Saline, Faulkner and Lonoke Counties and one in Hot Springs that serves Hot Spring and Garland Counties.
